What to bring to your appointment

When you come for your appointment please remember to bring the following:

1. A referral from your General Practitioner or Family doctor.

2. Your healthcare cards: Medicare, private health care card or Veteran's Affairs card.

3. All relevant x-ray reports, blood tests or other test results from your primary care or referring physician.

4. A list of all medications that you are currently taking.

5. A list of any drug allergies you have.

6. If you have had surgery or a colonoscopy elsewhere, please bring a copy of your operation / colonoscopy report.
